It's difficult to compare my career with the rest of the world becouse the way that my study program is implanted exist only in a few counties, in the most of the countries the people who wants to be a matron needs to study nursing and then specialize in obstetrics.
In Chile there are more or less about 9 obstetrics school, I'm studing in the oldest one. For this reason we know more or less the situation of matrons here in our country since this career exist in Chile.
In my country the topics related to reproductive health is completely related to matrons. In every public health centre there are a pool of matrons that helps people, and specifically couples or women that need help with their sexual life.
We are a country with neonatal mortality rates lowest in Latin America and in the world. In 30 years we got a neonatal infant mortality from 16.5% to a rate close to 4%. Similarly, perinatal maternal mortality is very low, close to 0,022%. We got that rates thanks to the proffesional rol that we have in the chilean health system, we are really indipendent from medics in the public system.
I think we have mainly two problems, in the private system for example, the majority of the deliveries are attended by obstetrics medics (gynecolgists). The problem is that now women are opting to have their babies with a cesarean and not by a natural delivery. Chile has the highest rates of cesarean in the world, about 24% or 28%, and the WHO (World Heath Organization) recommended only a rate of 10% to 15%. The main reason of that rate of cesareans is becouse women and medics decide to do a cesarean section for comfort. But in general comfort only for the medic, becouse they schedule when to do the operation and then they are free of work again, instead of a natural delivery, where the work can take about 12 hours. We have to remedy this situation becouse a natural delivery is less dangerous than a operation, the natural delivery is just a phisiologic process.

well said. I really do think alike with you regarding surgery.
ReplyDeleteI'm quite pro-natural birth, 'cause, if you think about it, c-section isn't birth, it's just an extraction, like when you cut out a tumor or something like that.
greetings!
